Hi there guys

Just started using breezingforms as the end products seems very cool But for the life of me am struggling to even make a simple form

Can anyone assist me with making an order form or maybe uploading a simple order form package that i can manipulate with my own data?

Im looking for something that has 8 columns across and as many rows as i can have:

- 1st column is static text with sub-product name and the 7 other rows are tick boxes.
- 1st row is static text and explains the main product details and sizes for the tick boxes below

Any help would be greatly appreciated I have attached an example of what im looking for.
