Not sure whats going on I made no changes what so ever to the form and it looks to work perfectly you can test it here:

Me either because in your version it works perfectly! In my version my second page has none of the entered details displaying and my email which gets directed to This email address is being protected from spambots. You need JavaScript enabled to view it. contains none of the entered details. Yet I received the email to This email address is being protected from spambots. You need JavaScript enabled to view it. with the test info I just entered. Could it be my template interfering? Is there any benefit to you packaging that version and me uploading? Thank you so much for all that you have done so far!!

\ok Your all set, I uploaded my version of the file (no changes were made) and then I re installed the software, my version works, but yours is not giving me the same results. I apologize for the wait. but your good to go.
