This is a simple contact form that includes a PDF download on the thank you page.

You can install and extend this form as desired, keeping the pdf download. Please leave the empty div on the last page (=thank you page) as it is used to populate with the link to the PDF after submission. If you change the thank you page text, switch to HTML mode to make sure not to loose this div, otherwise the download won't be visible.

Please do not remove the codes in before and end submit pieces if you intend to use your own custom pieces!

You can edit the PDF templates by editing the default attachment PDF template located in "/media/breezingforms/pdftpl/pdf_attachment.php" or create a custom template for this form by copying the default one and renaming it to "FORMNAME_pdf_attachment.php"

Installation:

1. Download an unzip the form
2. In BreezingForms go to Configuration => Package Installer
3. Upload the xml from the zip
4. Go to manage forms, select the right package, enter the form and test/edit it
