The VirtueMart Breezing Forms Plugin allows you to do either of these simply and easily. Configure it in minutes to either present a form to all customers, or only when certain products are purchased. The form becomes part of the checkout process, and the customer can't finish checkout until all of the required fields are filled in.

The VirtueMart Breezing Forms Plugin PRO lets you seamlessly integrate custom Breezing Form(s) into the VirtueMart checkout process.

PRO Version 3.0 - works with Joomla 2.5 and VirtueMart v2.0.x - $45.00 USD

Community Version 1.3 - works with Joomla 1.5 and VirtueMart v1.1.x only - FREE
